Track, review and manage all client form submissions

What is Forms Management?

The Forms page is your central hub for every form a client submits — questionnaires, vet release forms, emergency contacts, boarding agreements, and policy acknowledgements. You can review submissions, edit answers, mark forms as read, and track which clients still have outstanding mandatory forms.

How the forms system works

Forms arrive in an inbox-style layout with three views: Inbox (unread), Read, and Mandatory Tracker.

Inbox tab

New form submissions appear here. Click any form to view the full response. Viewing a form automatically marks it as read.

Read tab

Previously reviewed submissions are moved here. You can undo a read status to send a form back to the inbox.

Mandatory Tracker

See a per-client progress view of all mandatory forms. Quickly identify who has outstanding forms and send reminders directly.

Edit responses

If a client provides incorrect information (e.g., wrong vet details), you can edit the form on their behalf and save changes.

Medical history

View and edit detailed medical history submissions per pet — conditions, allergies, medications, and more.

View all forms per client

Click on any client's name to see every form they have submitted across all form types.

Tips

Check the Mandatory Tracker regularly — clients cannot be fully onboarded until all required forms are submitted.

Use the "Send Form Reminder" button to nudge clients who have outstanding forms — it sends a personalised email listing which forms are still pending.

The search bar filters by client name, email, and form type — use it to quickly find a specific submission.

Editing a form does not notify the client. If answers change significantly, consider reaching out directly.

The coloured chips next to each mandatory form show completion at a glance — green means complete, grey means pending.
